I created a Nerazim variant of the Warp Prism in a my campaign, the Phase Prism, I create the unit for both forms and I made a new ability for the change of forms, but for some strange reason.
The Phase Prism don't change form, when I use the Phasic form he just turn around itself without open, and even I don't touched in any way the Warp Prism units or abilities, but when i change form of it, the game create a Phase Prism inside the Warp Prism, and appear the portrait and the wireframe of the Phase Prism.
I countolled both the abilities, and they are all correct.

1. You make two new unit that can morph (phase <-> warp).
2. You make two new ability so that those units can morph.
3. The units didn't morph as intended.
Well, first thing first.
The morph ability didn't actually 'morph' a unit, but instead it force the unit to play morphing animation then subtly replace that unit with another.
For example if you look at units list there are 2 units for tank. Siege mode and unsiege mode.
The trick is, you have to make sure that the abilities to siege and unsiege correctly link the unit that you want to replace the unit in question.

I don't know if I will release this campaign, because is basically, SC2 with more units, more tech, and heroes, and I don't think Blizzard will like to see a redone of SC2 in SC2 XD
But if you don't know, I done a SC1 remake :) (I'm working in a new patch to solve some bugs right now)

Well, since that didn't work here is something else you could try.
1. Open the original prism unit in the editor.
2. Open the actor tab and write down / screenshot them.
3. Open your created / new units and their respective actor tab.
4. Compare them, make sure everything is in there.
5. If something is missing you will have to add them manually.
That should fix it.
