Add a validator either to the ability or to the desired effect (to the ability if its only castable on targets with 20% hp, to the effect, if it is castable on all targets, but the effect does only kick in, if the target has < 20% hp)
Maybe there is even a pre-made target < 20% hp validator; if not, create a new one. Look at the other hp validators for reference, you should be able to copy and modify one of them to your needs.
o_o and apply the behavior to every possible target? When you just can skip the requirement and the buff and check for the target life with a validator directly? Possible ;)
I thought you can add a validator to the ability, but autocast validator only determines, if the unit is targeted by autocast and the smart validator I do not know exactly. I would add it to the effect used by the ability.

Think of it as a finishing move or execute ability. That would only work with that condition. How can I do this?
